form, unless the client is a minor (under 18 yr old- then the legal guardian should complete form.)<\/p><p>We also ask that these forms be 100% completed as accurately as possible with no missing information. This is to allow proper time for your foot soak, and to go over the intake form with you, questions if need to ask, session goals etc with you prior to the beginning of your session.<\/p><p>We understand that issues can arise that may cause you to be late for your appointment. However, we ask that you call to inform us if this ever occurs so that we can do our best to accomodate you. Appointment times are reserved for each client, so oftentimes we cannot exceed that reserved time without making the next client late. For this reason, arriving after your appointment time may result in loss of time from your massage so that your session ends ar the scheduled time.
